Two excellent opportunities are available for registrars to join a newly established Hospital in the Home medical team at a major regional health service in Victoria.
These standalone 12-month positions are suitable for doctors at BPT2 or BPT3 level who wish to develop their skills in acute and ambulatory medicine while providing hospital-level care to patients in their own homes.
The successful registrars will receive dedicated consultant support, structured supervision and a personalised professional-development plan aligned with relevant Royal Australasian College of Physicians training requirements.
The Position
The Hospital in the Home service manages a diverse range of acute medical and specialty patients who can safely receive hospital-level assessment, treatment and monitoring outside a conventional inpatient ward.
Care is delivered through:
- A dedicated Hospital in the Home clinic
- Telehealth consultations
- Visits to patients’ homes
- Collaboration with inpatient and specialty teams
- Coordination with nursing and allied health services
Key Responsibilities
The successful applicant will:
- Assess and manage patients admitted to the HITH service
- Develop and review individualised treatment plans
- Monitor patients receiving acute treatment at home
- Conduct clinical reviews in the HITH clinic, by telehealth and in patients’ homes
- Identify clinical deterioration and arrange escalation or hospital admission when required
- Coordinate investigations, medications and follow-up care
- Liaise with inpatient, emergency and specialty medical teams
- Work collaboratively with nursing, allied health and community providers
- Maintain clear and accurate clinical documentation
- Support safe admission to and discharge from the HITH service
- Supervise and mentor junior medical staff
- Participate in clinical governance, education and quality-improvement activities
A professional-development plan will be developed with the Medical Director and tailored to the registrar’s experience, learning objectives and physician-training requirements.

Applicants should:
- Hold a recognised medical degree
- Hold current or eligible medical registration with the Australian Health Practitioner Regulation Agency
- Have postgraduate experience appropriate for appointment at registrar level
- Be undertaking or eligible to undertake Basic Physician Training at BPT2 or BPT3 level
- Possess sound acute medical assessment and management skills
- Demonstrate effective communication, organisation and teamwork
- Be capable of working across clinic, telehealth and home-based settings
- Demonstrate an interest in ambulatory care and innovative models of healthcare delivery
- Meet all occupational health and immunisation requirements, including the 2026 influenza vaccination
